If you have heart disease, the Erickson Advantage Champion (Health Maintenance Organization-Point of Service Special Needs plan) plan may be right for you. Enroll anytime.*

The Champion plan offers extra benefits beyond Original Medicare to help Erickson Living residents with certain medical conditions. Champion features low premiums of only $196 per month, which includes Part D prescription drug coverage.

The Erickson Advantage Champion plan is available to Erickson residents having a qualifying chronic care condition such as diabetes, coronary vascular disease and chronic heart failure.

Plans are insured through UnitedHealthcare Insurance Company or one if its affiliated companies, a Medicare Advantage organization with a Medicare contract. Enrollment in the plan depends on the plan’s contract renewal with Medicare.
